Weare seeking an experienced Clinical Lead Nurse Practitioner to join ourTeamCare program in Bronx, New York. The Clinical Lead NP will be assigned to aTeamCare site and provide comprehensive care to adult patients while serving asa clinical leader within the practice. Thisposition combines direct patient care with clinical leadership and carecoordination. In the absence of, or at the direction of, the Medical Director,the Clinical Lead NP will represent the Medical Director and provide clinicalleadership at the assigned location.

Schedule: Monday-Friday 8:30-5:30 (1 hour lunch)

Compensation& Benefits

• Base salary $162,000-$190,000 depending on experience, plus bonus

Requirements

• Graduateof an accredited Nurse Practitioner program and eligible for NYSED recognition.
• 2+years of clinical experience in primary care, home care, care management, orrelated settings.
• 1+year of experience caring for adult patients preferred.
• Current,unrestricted NY Nurse Practitioner and RN licenses.
• CurrentBLS certification.
• ActiveCAQH profile.
• CurrentMedicare and Medicaid provider IDs.
• AANPor ANCC Board Certification and current DEA registration.
• Strongclinical judgment, communication, analytical, and organizational skills.
• Proficiencywith Microsoft Office and electronic clinical systems.
• Bilingualskills a plus
